BP Deepwater Horizon Explosion: Failure of Risk Management
The official federal report on the BP Deepwater Horizon blowout faults BP's failure to assess the risk of the actions that led to the blowout. BI has the story:
“The blowout at the Macondo well on April 20, 2010, was the result of a series of decisions that increased risk and a number of actions that failed to fully consider or mitigate those risks,” the report said. Read more

"What is Risk Management?"
That question always makes me think of George Costanza as he struggled with his task to explain a "Risk Management" textbook to the Yankees upper management. He could not read the book because he could not stand the sound of his own voice in his head. So, naturally, he pretended to be blind and had a free-service translate the book into an audiobook.
